结直肠癌“防治保康”中西医并重理论与实践模式研究＊

近三十年来,我团队致力于结直肠癌研究,初步建立防治保康中西并重模式,并付诸实践。应用中医之道：从建立多学科肿瘤治疗团队向建立多学科肿瘤康复团队的转变;用中医理论指导下生活方式的改变,以推进肠镜检查为结局,最终找出高危人群进行二级预防为目标;应用中医之术：以健脾补肾解毒为大法的结直肠癌全程治疗策略和多学科合作模式,为结直肠癌患者提供全程康复治疗,为单病种中西并重防治保康做出示范。借助于北京市中医肿瘤防治办公室和大数据创新实验室进行实践,在预防上积极探索以慢病管理APP为抓手,乡村干部、镇卫生院、区中医院为三级联动的结直肠癌中西医并重预防模式并向其他慢病扩展。在治疗方面,与北京肿瘤医院合作,在结直肠癌中西并重全程管理治疗模式中取得进展。在我院开展的中西医结合肿瘤康复多学科门诊模式标志着我国肿瘤康复事业前进了一大步。而在大数据分析上：团队对北京市三甲中西医医院住院病历的探讨,则能够从侧面看到目前的现状和存在的问题。本系列组稿将从多方面展现上述命题。

Establishment and Practice of the Mode of "Preventing, Treatment and Rehabilitation" of Colorectal Cancer with Integrated TCM and Western Medicine

Nowadays, the specific combination of traditional Chinese medicine (TCM) and western medicine in the prevention, treatment and rehabilitation of cancer has initially had the characteristics of China. TCM was born in the traditional Chinese culture during the past 5000 years. The blending of TCM and modern medicine created a unique medical team of integrated TCM and western medicine in China. Over the past 30 years, our team has been committed to the research of colorectal cancer, initially established a mode of "preventing, treatment and rehabilitation", which attached equal importance to TCM and western medicine, and put it into practice. Applying the theory of TCM: The change from treating diseases to treating people leads to the transformation of team establishment from multidisciplinary tumor treatment to multidisciplinary tumor rehabilitation. Adjust measures to the conditions according to place, time and people, and take ''correspondence between nature and man'' as the key link. Under the guidance of TCM theory, use herbs or non-medication therapy to treat the relevant symptoms of colorectal cancer precancerous lesions, as well as change of lifestyle. Promote enteroscopy, and finally identify high-risk groups for secondary prevention; Applying the therapy of TCM: Establish a green ward, a whole course of treatment strategy in colorectal cancer, with strengthening the spleen, tonifying the kidney and detoxifying as the main method, and multidisciplinary cooperation model. Provide the cancer survivorship care team and treatment environment for cancer patients, contribute to the "preventing, treatment and rehabilitation" mode of colorectal cancer with integrated TCM and western medicine, provide a demonstration of attaching equal importance to TCM and western medicine in the prevention and treatment of single disease.A national "13th Five-Year Plan" research has been conducted to obtain high-level medical evidence of TCM effect in colorectal cancer, with the method of strengthening the spleen and tonifying the kidney. In the Beijing municipal administration of traditional Chinese medicine and xiyuan hospital leadership support, with the help of Beijing Chinese medicine tumor prevention office and big data innovation laboratory for practice, on prevention, based on the Beijing administration of traditional Chinese medicine 30 leading rural project practice, the team actively explore to slow disease management APP as the gripper, rural cadres, town hospitals, district hospital for tertiary linkage of colorectal cancer of traditional Chinese medicine and western medicine prevention mode and expand to other slow disease. In terms of treatment,The major difficult issues in the 13th Five-Year Plan with Beijing Cancer Hospital have made progress in the whole-process management and treatment mode of both Chinese and Western treatment of colorectal cancer. The multidisciplinary outpatient mode of integrated traditional Chinese and western medicine in our hospital marks a big step forward in tumor rehabilitation in China.On the big data analysis, The team''s discussion of the inpatient medical records in Beijing a Traditional Chinese and Western Medicine Hospital can see the current situation and existing problems from the side.
