| 摘 要： | 目的对家庭保健员计划实施的效果进行评价。方法按入选标准在北京市城八区入选1万名左右的家庭保健员，限一户一人，对其进行慢性病防治知识的培训和管理，再由其担负起对其他家庭成员健康教育、生活指导以及医患互动联络的作用，半年后评价家庭保健员及其家庭成员慢性病防治知识和技能的改善情况。结果共6137个家庭参与了干预前后的问卷调查，其中资料完整者11192人（4723名为家庭保健员），平均年龄（53．2±15．8）岁，男4980人，占44．5％。干预后，慢性病防治总得分从66．2分增加到85．5分，增加了19．4分。“行为危险因素防治知识”得分增幅最大，从61．8分增加到了85．2分，增加了23．3分。单因素分析显示，文化程度较低、家庭成年人口少、中老年人群的得分增长幅度最大；多因素分析显示，基线得分低、家庭成年人口较多、青壮年人群的干预效果更好。结论北京市家庭保健员计划项目效果显著，针对基线慢性病防治知识得分低的人群效果更佳；如果没有条件开展基线调查，可以文化程度较低的家庭作为工作重点。 |
