脑静脉窦血栓形成的临床和影像学诊断价值探讨

目的探讨脑静脉窦血栓形成的临床和影像学诊断价值。方法对12例脑静脉窦血栓形成患者的临床及影像学资料进行分析。结果患者多表现为颅内高压征象,伴或不伴局灶神经功能异常。头颅CT未见特征性表现,MRI仅5例示静脉窦异常信号,头颅磁共振静脉成像（MRV）12例均显示病变静脉窦狭窄或者闭塞。结论对临床上主要表现为颅内高压征象伴或不伴定位体征的高危患者头颅CT不能明确诊断,应及时行MRI结合MRV检查可快速明确诊断脑静脉窦血栓形成,并避免误诊误治。MRI及MRV简便易行、快捷无创,易于临床推广,可作为诊断脑静脉窦血栓形成的首选方法。

Investigation of the Clinical Manifestation and the Diagnostic Value of Imaging of Cerebral Venous Sinus Thrombosis

Objective To investigate the clinical manifestation and the diagnostic value of imaging of cerebral venous sinus thrombosis（CVST）.Methods 12 cases of patients with CVST were analyzed with the help of clinical features and imaging signs.Results Most patients had signs of intracranial hypertension.Some patients manifested symptoms of whole brain or focal neurological deficits.Cranial CT demonstrated no characteristic features,while MRI showed only five cases of venous sinus abnormal signal.Cranial magnetic resonance venography（MRV） displayed that lesion venous sinus stenosis or occlusion existed.Conclusion The high-risk patients of intracranial hypertension,with or without clinical positioning signs,should take MRV check as soon as possible to quickly and clearly diagnose the formation of cerebral venous sinus thrombosis and to avoid incorrect diagnosis and treatment.It is proved that MRV is easy to take,fast and non-invasive,and clinically recommendable.Therefore,it can be used as the priority method in diagnosing cerebral venous sinus thrombosis.
