乳头溢液的超声鉴别诊断初步探讨

目的 探讨超声检查对乳头溢液的鉴别诊断价值。方法 应用实时高频超声对55例乳头溢液患者术前进行检查，检查结果与手术病理结果相对照。结果 55例乳头溢液最主要的病因是乳腺导管内乳头状瘤（72.73%），其次是乳腺囊性小叶增生（16.36%）、乳腺导管癌或导管内乳头状瘤恶变（7.27%）及乳腺导管扩张症（3.64%）。超声诊断准确率、敏感性的特异性分别为75.00%、67.27%和88.28%。结论 超声检查能提供准确的病变部位、乳管扩张情况及其腔内的微小占位性病变的信息，可为临床制定合理的治疗措施提供重要的参考依据。也可为乳管造影术、纤维乳管镜检查提供了定位信息，有助于提高其检查成功率。

Priliminary study of ultrasonic differential diagnosis of nipple discharge

Objective To study the value of ultrasound to differentially diagnose nipple discharge.Methods Fifty-five cases with nipple discharge were examined preoperatively with real-time high-frequency ultrasound. The ultrasound findings were compared with those of surgery and pathology.Results The main cause of nipple discharge was intracanallicular papilloma ( 72.73 %), the others were cystic hyperplasia of breast( 16.36 %), intracanallicular papillocacinoma( 7.27 %) and mammary duct ectasia( 3.64 %). The accuracy,sensitivity and specificity of ultrasonic differential diagnosis of nipple discharge were 75.00 %, 67.27 % and 88.20 %,respectively.Conclusions Ultrasound easily provides the information about nipple discharge such as location, degree of ductal dilatation and tiny-lesions in the duct for surgeons to selecte a proper treatment for every case.
