电针镇痛的累积效应与脑内孤啡肽前体和前阿黑皮素基因表达的关系

摘    要:目的:探讨累加电针治疗神经源性痛大鼠的作用机制。方法:Wistar大鼠随机分为正常对照组,慢性压迫性损伤(CCI)组,CCI+EA2(天)D组,CCI+EA2(周)W组,去卵巢(OVX)+CCI组,OVX+CCI+EA2D组,OVX+CCI+EA2W组。去卵巢大鼠在手术后每天颈部皮下注射D-半乳糖,连续45天造成神经记忆损伤。结扎坐骨神经造成慢性神经疼痛模型,电针双侧"足三里"-"阳陵泉"穴1次/D,分别电针2D、2W。用RT-PCR的方法检测下丘脑前阿黑皮素(POMC)mRNA和海马、下丘脑孤啡肽前体(PPPNOC)mRNA的表达。结果:与正常对照组比,CCI组下丘脑POMC mR-NA表达水平显著升高(P<0.05),海马及下丘脑PPNOC mRNA表达水平也显著升高(P<0.05)。与CCI组比,CCI+EA2W组POMC mRNA表达水平进一步显著上调(P<0.05),而PPNOC mRNA表达水平显著降低(P<0.05)。在OVX+CCI动物上,与OVX+CCI组比,OVX+CCI+EA2W组下丘脑POMC的表达量明显上调(P<0.05),而OVX+CCI+EA2D和OVX+CCI+EA2W海马PPPNOC mRNA的表达显著下调(P<0.05),但后两组下丘脑PPPNOC mRNA的表达无显著变化(P>0.05)。结论:电针对慢性神经痛大鼠的累积性镇痛效应与下丘脑POMC mRNA表达的上调和海马PONC mRNA表达的下调密切相关;记忆损伤可减弱电针上调下丘脑POMC基因的表达。
