全氟化碳液辅助脱位晶状体摘除联合后房型人工晶状体缝线固定术

摘    要:目的 探讨全氟化碳液辅助脱位晶状体摘除联合后房型人工晶状体缝线固定术的临床疗效和安全性。方法 对18例(18只眼)白内障超声乳化术中晶状体脱入玻璃体腔的患者行全氟化碳液辅助脱位晶状体核摘除联合后房型人工晶状体缝线固定术,观察术后并发症,检测术后视力,随访6~24个月。结果 术后裸眼视力在0.1~0.4者4眼(22.2%),0.5~0.9者13眼(72.2%),1.0以上者1眼(5.6%)。主要手术后并发症:高眼压,前房积血,玻璃体腔积血,脉络膜脱离,前房炎症反应,角膜水肿等,术后1~2周均得到控制,未出现黄斑囊样水肿及视网膜脱离等并发症。结论 该术式是一种安全有效、视力恢复满意的治疗方法。

Abstract:Objective To explore the safety and therapeutic effectiveness of the combination of dislocated lens surgery with transscleral sulcus fixation of posterior chamber intraocular lens using perfluorocarbon liquid.Methods Eighteen eyes of lens dislocation into vitreous cavity during phacoemulsification were treated with the combined operation.Postoperative complications and visual acuity were observed.All the cases were followed up for 6-24 months.Results The uncorrected visual acuities were 0.1-0.4 in 4 eyes(22.2%),0.5-0.9 in 13 eyes(72.2%) and 1 eye was >1.0(5.6%).The main operative complications including ocular hypertension,anterior chamber hemorrhage,vitreous hemorrhage,transient choroidal detachment,corneal edema and anterior chamber inflammation reaction were controlled within 1-2 weeks postoperatively.No severe long-term complications occurred.Conclusion The combined operation can be an effective and safe clinical method to the visual rehabilitation.
