卡维地洛对充血性心力衰竭及脑钠肽水平影响的临床研究

目的探讨卡维地洛对充血性心力衰竭患者血浆脑钠肽（brain natriuretic peptide,BNP）水平及对左室功能的影响。方法将92例充血性心力衰竭患者随机分为卡维地洛组（46例）和对照组（46例）。对照组用血管紧张素转化酶抑制剂、利尿剂和地高辛,卡维地洛组在对照组药物治疗基础上加用卡维地洛,治疗12周后观察患者BNP和超声心动图的变化。结果与对照组比较,卡维地洛组BNP水平下降程度更明显（P〈0.05）,左室舒张末内径减小程度更明显（P〈0.05）,左心室射血分数（LVEF）增加更明显（P〈0.05）。结论卡维地洛治疗充血性心力衰竭效果显著,可更有效地降低血浆中BNP水平,增加LVEF、改善左室功能。

Clinical observation on the effects of carvedilol on plasma level of brain natriuretic peptide and cardiac function of patients with congestive heart failure

Objective：To research the effect of carvedilolon the levels of plasma brain Natriuretic Peptide（BNP） and left ventricular function of patients with Congestive heart failure（CHF）.Methods： 92 patients with Congestive heart failure were randomly divided into two groups： The control group（with digoxin,diuretic and ACE,n=46） and the carvedilol group（with carvedilol in addition to control therapy,n=46）.The effects were observed after 12 weeks treatment.The changes in levels of BNP and echocardiographic indices were measured before and after treatment.Results： Level of BNP was significantly decreased in carvedilol group,left ventricular ejection fraction（LVEF） were significantly increased in carvedilol group compared with control group.Conclusion： Better curative effect will be achieved on patients with CHF who take carvedilol in addition than those who only take digoxin,diuretic and ACEI Carvedilol can decrease plasma BNP concentrations and mi prove cardiac function.
