甲泼尼松龙治疗AECOPD用药时间的临床探讨

目的 探讨甲泼尼松龙治疗AECOPD用药时间.方法 选择确诊为AECOPD患者60例,随机分为3组,激素阶梯组、激素短期组、对照组.激素阶梯组予甲基强的松龙40 mg静脉点滴,3～5天后减半,治疗3天后,改强的松10 mg,持续3天.激素短期组予甲基强的松龙40 mg短期静脉点滴3～5天,症状缓解后直接停用.对照组常规治疗.比较三组治疗前、治疗后5天、治疗后10天的动脉血气PaO2及呼吸困难评分、症状体征缓解消失的时间、住院天数.结果 激素阶梯组在治疗10天后PaO2和呼吸困难评分、症状、体征恢复时间、住院时间分别与激素短期组、对照组比较有统计学差异(P<0.05).结论 激素阶梯治疗能较好改善AECOPD患者症状,缩短住院日,临床值得推广.

Clinical analysis of the different medication methods of methylprednisolone on AECOPD

Objective To evaluate the different medication methods of methyprednisolone on Chronic Obstructive Pulmonary disease in acute texacerbation stage(AECOPD) and therefore improve the therapy of AECOPD.Methods 60 patients diagnosed with AECOPD were divided randomly into three groups: glucocorticoid step term group,glucocorticoid short term group and control group.The patients of glucocorticoid step term group were treated with 40mg methyprednisolone by intravenous drop infusion for three to five days,then the dosage of methyprednisolone decreased to 20 mg for three days,followed by 10 mg for another three days.The cases of glucocorticoid short term group were treated with 40 mg methyprednisolone by intravenous drop infusion for three to five days.The cases of control group received conventional therapies.Furthermore,data such as arterial blood PaO2,the score of dyspnea,the relief time of main symptoms and signs,as well as the hospital days were collected and evaluated between the three groups.Results Compared with other two groups respectively,the data of the glucocorticoid step term group showed statisticallly significant differences in all the measurements detected after 10 days of treatment(P<0.05).Conclusion The glucocorticoid step therapy can markedly improve the symptoms and shorten the hospital days of the patients of AECOPD.
