支气管肺泡灌洗联合BiPAP治疗AECOPD伴呼吸衰竭的临床研究

目的评价支气管肺泡灌洗(BAL)联合双水平无创正压通气(BiPAP)治疗慢性阻塞性肺疾病急性加重(AECO-PD)伴呼吸衰竭患者的疗效及安全性。方法 42例AECOPD伴呼吸衰竭的患者在接受常规药物治疗的同时联合BiPAP治疗,治疗组除上述治疗外给予BAL术。比较两组治疗前后生命体征、动脉血气分析及有效率的变化。结果两组治疗后心率、呼吸频率、PH、PaCO2较治疗前改善,治疗24 hPaO2较对照组改善。结论 AECOPD伴呼吸衰竭患者在BiPAP治疗的同时采用BAL能改善患者症状,能够有效纠正其缺氧和二氧化碳潴留。

Clinical research for treatment Acute Exacerbations of Chronic Obstructive Pulmonary Disease with respiration failure by Bilevel positive airway pressure ventilation Combined with Bronchoalvoelar Lavage

Objective To discuss the clinical efficacy and adverse effect of bilevel positive airway pressure ventilation(BiPAP) combined with bronchoalvoelar lavage(BAL) in treating respiration failure of acute exacerbations of chronic obstructive pulmonary disease(AECOPD).Methods A total of 42 patients with AECOPD complicated with respiratory failure were all treated with conventional therapy and BiPAP ventilation.Patients in treatment group were added to BLA.Observations were made on the clinical symptoms and changes of arterial blood gas before and after 2 h,24 h ventilation.Results Compared with control group,all indices,except PaO2 at 2 h,were improved.The indices observed in treatment group were more significantly improved than those in control group after 24 h treatment.Conclusion The clinical symptoms and changes of arterial blood gas were improved in patients with respiration failure of AECOPD after BiPAP ventilation combined with BAL.
