25例结核性支气管狭窄死亡病例分析

目的分析结核性支气管狭窄病例死亡原因,探讨治疗措施的相关问题。方法回顾分析我院2008年6月至2009年12月收治的结核性支气管狭窄死亡病例25例及临床资料。结果 25例患者均因弥漫性气管、支气管狭窄致急性呼吸窘迫综合征死亡;其中12例行人工气道机械通气等治疗,13例行面罩式无创机械通气等治疗。结论 25例气管狭窄死亡病例,死亡原因均为弥漫性气管、支气管狭窄致急性呼吸窘迫综合征死亡,行机械通气等抢救措施治疗均无效,因此寻求新的抢救措施势在必行。

Clinical analysis of death in 25 cases of tuberculous bronchial stenosis

Objective To investigate the cause of death in the patients with tuberculous bronchial stenosis and the related problems during the treatment.Methods The clinical data of 25 patients suffering from bronchial stenosis caused by tuberculosis treated in our hospital from June 2008 to December 2009 were collected and retrospectively analyzed.Results The total 25 patients,12 cases were treated with endotracheal mechanical ventilation(ETMV) and 13 cases with non-invasive pressure support ventilation(NIPSV),died of acute respiratory distress syndrome(ARDS) caused by diffuse trachea or bronchus stenosis.Conclusion Our data suggest that the main cause of death in patient with tuberculous bronchial stenosis was acute respiratory distress syndrome(ARDS),mechanical ventilation was not an effective therapy.Further study of the treatment is urgent needed.
