法舒地尔改善慢性肺源性心脏病患者右心形态功能的研究

目的 对接受法舒地尔治疗的肺心病患者进行测量及随访.方法 50例慢性肺源性心脏病右心功能不全患者,随机分为治疗组和对照组,每组各25例,治疗组静脉滴注生理盐水100 ml+盐酸法舒地尔30 mg,每日1次,均治疗14 d,同时常规治疗.结果 治疗后1个月随诊,两组右室舒张末期内径均较治疗前减少,治疗组较对照组显著减少,两组右室收缩末期容积无显著改变,组间比较无明显变化,治疗组肺动脉压力较治疗前及对照组显著减少,治疗组右室Tei指数较治疗前及对照组显著增加.结论 法舒地尔对改善慢性肺源性心脏病患者右心形态及功能,短期效果确切.

The Research Of Using Fasudil To Improve The Right Ventricular Function And Morphology Of Patients Undergone The Chronic Cor Pulmonale

Objective We measured the patients who were undergone the chronic cor pulmonale before treatment and one month follows-up to evaluate the effect of morphological and functional changes of right ventricle.Methods Fifty patients were divided equally into treatment group and comparing group randomly.Patients in the treatment group were given fasudil intra-vena once a day for 14 days beside the ordinary treatment while the ones in the comparing group were only given ordinary treatment.Echocardiography examinations were taken as soon as being inpatient and one month after treatment.Exam items includes EDV(end-dialotic volume)、ESV(end-systolic volume)、pulmonary pressure、ICT(iso-contraction time)、IRT(iso-relaxation time) and ET(ejection time) of right ventricle were measured on the basis of blood spetrum of mitral valve and tritral valve in the lead of electrocardiography,then we use the three items to calculate right ventricles’Tei indexes.Result In one month follows-ups two groups’RVEDVs reduced compared with the ones before treatment meanwhile treatment group’s indexes reduced compared with comparing group remarkably.RVESVs had no significant change successively and between the two groups they had no significant differences.Pulmonary pressures of treatment group had significant changes successively and they reduced compared with comparing group.Tei indexes’ changes were similar with the Pulmonary pressures’.Conclusion Fasudil has the exact short-term effect to improve the right ventricular function and morphology of patients undergone the chronic cor pulmonale.
