插管联合丝裂霉素C治疗老年人泪道阻塞及慢性泪囊炎

目的观察插管联合丝裂霉素C治疗老年人泪道阻塞及慢性泪囊炎的效果。方法对45例57眼用甲硝唑注射液冲洗泪道和行泪道探通后,将硬膜外麻醉导管插入泪道,并将0.4 mg/ml丝裂霉素C与等剂量透明质酸钠混合的胶体注入阻塞部位。术后用甲硝唑注射液(一般临床供静脉注射所用)及α-糜蛋白酶进行泪道冲洗。视病情15~45 d拔管。结果随访6~12个月,本法治疗泪道阻塞和慢性泪囊炎有效率均达到85%以上,两者疗效差异无统计学意义。结论插管联合丝裂霉素C能较有效治疗老年人泪道阻塞及慢性泪囊炎。

Intubation treatment of lacrimal ducts obstruction and chronic dacryocystitis with mitomycion-C in elderly patients

Objective To observe the effect of nasolacrimal intubation treatment of lacrimal ducts obstruction and chronic dacryocystitis with mitomycion-C in elderly patients.Methods Forty-five patients(12 males,33 females;aged 55-76 years),57 eyes with lacrimal ducts obstruction or chronic dacryocystitis,after syphilis of lacrimal passageswith arilin solution for 5 minutes and probing,were intubated segmental eqidural catheter into the lacrimal ducts,and were injected mixture colloid of 0.4 mg/ml mitomycin-C and isodose sodium hyaluronate into the obstruction location.Irrigate lacrimal ducts with arilin solution and avazyme postoperatively.Extubation was performed after 15-45 days.Results Follow-up ranged from 6 to 12 months.This treatment was effective in more than 85% patients.There was no significant difference between lacrimal ductsobstruction group and chronic dacryocystitis group,P=0.085.Conclusion Segmental eqidural cathete nasolacrimal intubation with mitomycin-C was one of the comparatively ideal therapies for lacrimal ducts obstruction and chronic dacryocystitis in elderly patients.
