放射治疗43例非小细胞肺癌脑转移疗效分析

目的观察放射治疗对非小细胞肺癌（NSCLC）脑转移的疗效,探讨获得更好疗效的方法。方法2005年5月～2006年12月43例NSCLC脑转移患者接受了放射治疗,根据原发灶控制情况和有无颅外转移病灶进行分组观察,对不同生存情况进行比较。结果所有顺利完成放疗,无严重副反应和并发症发生。放疗后颅内病灶控制率为81.1%,神经系统症状缓解率为75.7%,KPS评分显著上升。全组生存时间2～36个月,中位生存时间8.6个月,半年、1年、2年生存率分别为59.5%、27.0%、5.4%。RPA1级和2级中位生存期分别为23和9.5个月,2组患者生存期有明显差异。结论3DCRT联合WBRT可有效缓解非小细胞肺癌脑转移患者症状,并延长生存期和提高生活质量。

Results of radiotherapy in 43 non-small cell lung cancer patients with brain metastasis

Objective To observe the treatment outcome of 3-dimension conformal radiotherapy （3DCRT） in non-small cell lung cancer （NSCLC） patients with brain metastasis and explore the more effective treatment for those patients. Methods From May, 2005 to December, 2006, brain radiotherapy was carried out in 43 patients who suffered from NSCLC with brain metastasis, they were divided into two groups according to control of primary site and extracranial metastasis, comparing their survival and quality of life. Results All the patients finished 3DCRT and no serious adverse reaction and complication was found. Local control rate was 81.1%, the nervous system symptom relief rate was 75.7% and KPS score was raised significantly. Survival time ranged from 2 to 36 months, median survival time was 8.6 months, and 6-month, 1- and 2-year survival rate was 59.5%, 27.0% and 5.4%, respectively. Conclusions 3DCRT with WBRT is one of the effective palliative treatments for the patients suffered from lung cancer with brain metastasis, which may prolong their survival and improve their quality of life.
