复发性嗜铬细胞瘤的诊治分析

目的:提高复发性嗜铬细胞瘤的诊治水平.方法:总结1973～2003年术后复发的嗜铬细胞瘤18例,占同期嗜铬细胞瘤的7.9%(18/227).结果:经临床及病理证实复发原因为手术种植或残留肿瘤组织的复发、恶性肿瘤的转移及良性肿瘤的多发.肾上腺外嗜铬细胞瘤易多发或转移,转移位于无嗜铬组织的部位,多发部位常在肾上腺外.结论:嗜铬细胞瘤复发原因多种.多发及肾上腺外肿瘤恶性倾向较大应密切观察.为降低复发率,术前应明确肿瘤是否为多发,术中应避免肿瘤组织的种植或残留,对有恶性倾向的肿瘤应行根治性切除.

The Diagnosis and Treatment of Recurrent Pheochromocytoma

Objective: To improve the diagnosis and treatment of recurrent pheochromocytoma. Methods: A total of 18 cases with recurrent pheochromocytoma, treated during 1973 to 2003, were studied. Results: The recurrence was due to the implantation or remain of tumor, the metastasis of malignant tumor and the multifocal occurrence of benign one. Extra-adrenal pheochromocytomas tended to occur at several sites or metastases. The metastatic places occurred in no pheochromatic tissues, and the recurrent benign tumors were liable to grow at extra-adrenal regions. Conclusions: Not all recurrent pheochromocytomas are malignant tumors. Multifocal occurrence and extra-adrenal tumor with considerable malignant tendency should undergo intensive surveillance. To minimize the recurrence, we should avoid the implantation or remain of the tumor and undergo radical resection in tumor with malignant tendency.
