血清IL-21水平在川崎病发病机制中的作用

摘    要:目的通过检测川崎病(KD)患者血清白细胞介素-21(IL-21)的表达水平,探讨其水平变化与KD患者发病中的作用以及和血沉、IgA、冠状动脉扩张程度的相关性。探讨IL-21水平与KD的发病的关系,研究KD的发病机制,为KD的防治提供新的策略,从而加强对川崎病心血管及靶器官的保护。方法选取符合2002年修订的KD诊断标准KD患儿,采用酶联免疫分析法,检测血清IL-21水平,同时行心脏超声及其他免疫指标检测,并与其他临床指标进行统计学分析。结果①KD患者血清IL-21明显高于正常患儿,特别急性期及冠脉损伤组更为明显,差别具有统计学意义(P<0.05);②KD患儿血清IL-21水平与ESR及IgA,冠状动脉扩张程度存在明显正相关(r=0.621,P<0.05;r=0.502,P<0.05;r=0.583,P<0.05)。结论 KD患儿血清IL-21水平明显高于健康对照组,且急性期血清IL-21水平与ESR、IgA、冠状动脉扩张程度之间均存在正相关,说明血清IL-21水平与川崎病密切相关,可能参与了川崎病的发生发展,且在川崎病冠状动脉变化过程中起到了重要作用。

Abstract:Objective To investigate the relation between the change of serum IL-21 level and other laboratory targets by detecting serum IL-21 level from patients with Kawasaki disease (KD) and explore the role of IL-21 in KD, in order to research KD's pathogenesis and supply new method to protect coronary artery. Methods The level of serum IL-21 from KD patients diagnosed according to 2002 revised international classification criteria for KD were detected by enzyme-linked immune sorbent assay (ELISA) respectively, we detect the level of IL-21 and analyses the result. Meanwhile, the degree of coronary artery dilatational IL-21 were detected by Pearson. Results The level of serum IL-21 in patient KD was obviously higher than that in healthy control (P〈0.05) . In addition, the level of serum IL-21 level in acute KD patients showed positive correlation with ESR (r=0.621, P〈0.05) and IgA level (1-=0.502, P〈0.05) and the degree of coronary artery dilatation (1"=0.583, P〈 0.05) . Conclusion The level of serum IL-21 in patient KD were obviously higher than that in healthy control. The evident increase of serum IL-21 level in KD patients has positive correlation with the levels of ESR and IgA and the degree of coronary artery dilatation, which suggests that IL-2 lmay play a activity in KD. It takes part in the development of KD, and is relation to coronary artery dilatation.
