王好古辨治“三阴证”学术思想探析

王好古在张元素脏腑辨证学说和李东垣脾胃内伤理论的启发和影响下,注重脏腑虚损和伤寒三阴虚证研究,其确立阴证学说的范畴为《伤寒论》的太阴、少阴、厥阴三经的证候,即“伤寒内感三阴经”.其将面赤、手足振摇、腰腿沉重作为阴证的早期证候,将身如被杖、色青黑、手足踡而卧、恶闻人与语、昏昏欲寐、目白睛而赤等作为阴证的晚期证候.认为三阴证的病机为元阳中脱,治疗上重视肾气虚寒,常予脾肾双补之剂,且讲究服药方法.

Wang Haogu' s Academic Idea of Three-yin Syndrome Differentiation and Treatment

Inspired and influenced by Zhang Yuansu’s theory of zangfu syndrome differentiation and Li Dongyuan’s theory of internal impairment of spleen and stomach,Wang Haogu paid attention to the research on the visceral impairment and three-yin deficiency syndromes.His establishment categories of yin syndrome theory are the syndromes of Taiyin,Shaoyin and Jueyin,i.e.the three-yin syndromes of febrile internal pathogens.The early symptoms are the red face,shaking hand and heavy waist and legs,while the advanced symptoms are the painful body,black complexion,sleep with curled hand and feet,aversion to other people’s talk,tendency to asleep and white and red eyes.He thought that the pathogenesis of the three-yin syndrome was the collapse of the original yang,and in the treatment the deficiency and cold of kidney-qi should be stressed,the supplementing drugs for the spleen and kidney be used and the administration method be paid attention to.
