调强适形放疗联合吉西他滨化疗治疗局部晚期胰腺癌

摘    要:目的:探讨调强适形放疗联合吉西他滨治疗局部晚期胰腺癌的疗效和毒性反应。方法:2009年1月至2010年5月共收治局部晚期胰腺癌患者21例,采用调强适形放疗(IMRT),总量50.4~56 Gy;同步化疗方案为:吉西他滨1 000 mg/m2,第1、8、15天静脉滴注,28 d为1周期,与放疗同时开始,同步放化疗结束后1个月巩固化疗2~4周期。结果:所有患者均完成同步放化治疗。有效率为57.14%,局部控制率为90.48%。疼痛缓解率85.71%(18/21),生活质量明显改善,无治疗相关性死亡;1、2年生存率分别为66.67%和23.85%,中位生存期为18个月。结论:调强适形放疗同步吉西他滨治疗局部晚期胰腺癌疗效显著,能提高局部控制率,延长生存期,缓解疼痛,提高生活质量,且毒副反应能够耐受。

Abstract:Objective: To explore the efficacy and adverse reaction of intensity modulated radiotherapy combined with gemcitabine for locally advanced pancreatic carcinoma patients.Methods: From January 2009 to May 2010,21 patients were treated by IMRT(50.4~56 Gy) and concurrent chemotherapy using gemcitabine 1 000mg/m2(d1,d8,d15),28 days in each cycle.After CCRT,2-4 cycles of the same program for consolidation chemotherapy were used one month later.Results: All of the patients finished the CCRT.The total response rate was 57.14%.The local restraining rate was 90.48%.The pain-alleviation rate was 85.71%.Quality of life was improved without any treatment related death.The survival rate for 1,2 years was 66.67%,23.85%,respectively.The medium overall survival was 18 months.Conclusion: Intensity modulated radiotherapy combined with gemcitabine for advanced pancreatic carcinoma is effective and well-tolerated in patients with advanced pancreatic carcinomas.
