动态监测降钙素原对肝移植患者感染及预后判断的意义

目的:探讨动态监测降钙素原(PCT)对肝移植患者术后感染及预后判断的临床意义。方法:检测20例健康对照和63例资料完整的肝移植患者术前1天、术后第1、3、5、7天血清PCT水平,分析PCT变化与肝移植感染及预后之间的关系。结果:肝移植患者术前PCT水平与健康对照组差异有统计学意义(P<0.05),术后第1天PCT水平显著升高(P<0.05);合并感染组升高幅度大于非感染组(P<0.05),且1周内各时点血清PCT水平均高于非感染组,差异有统计学意义(P<0.05);感染控制后两组差异无统计学意义(P>0.05)。结论:动态监测PCT水平变化可用于肝移植患者术后感染的监测,指导抗生素应用,协助预测肝移植患者预后。

The significance of the dynamic monitoring of procalcitonin in liver transplantation patients' with postoperative infection and predicting prognosis

Objective: Study the clinical significance of the dynamic monitoring of procalcitonin in liver transplantation patients’ postoperative infection and predicting prognosis. Methods: To examine the PCT level of 20 cases of normal controls and 63 cases of patients with complete data of liver transplantation at the day before surgery and after surgery 1,3,5 and 7d respectively and to analyze the relationship between the PCT changes and the liver transplantation infection and prognosis. Results: The difference between the preoperative PCT level of liver transplantation patients and healthy control group has statistics significance(P< 0.05),and after the first day,the PCT level increased remarkably (P<0.05).The combined infection group is increasing greater than the non-infected group (P<0.05),and within 1 week,at each point,the PCT level in serum are higher than those in non-infected group,and the difference was statistically significant(P<0.05);After controlling infection,the two groups had no significant difference(P>0.05). Conclusion: Dynamic changes monitoring in PCT levels can be used to monitor the preoperative inflection of the liver transplantation patients,guide the antibiotic application,and assist in the prediction of the prognosis of patients with liver transplantation.
