摘    要:目的:探讨早产儿生后3 d内血气分析值对早产儿视网膜病(retinopathy of prematurity,ROP)的影响。方法:按照我国卫生部在2004年制定的《早产儿治疗用氧和视网膜病变防治指南》中所规定的筛查对象,396例患儿生后最初3 d血气分析被收集,统计分析患儿的PaO2、PCO2及pH值对ROP影响。结果:ROP患儿中占下四分位数PaO2、pH值比率、上四分位数PaO2、PCO2比率明显高于无ROP患儿,有统计学意义(P<0.05),对3期以上ROP患儿与无ROP患儿不同出生体重、胎龄、血气分析四分位值例数,经逻辑回归分析发现,出生体重小于1000 g、出生胎龄小于27周早产儿,与血气分析上下四分位数PaO2、上四分位数PCO2、下四分位数pH值有统计学意义(P<0.05),提示是发生ROP高危因素。结论:低PaO2、pH值及高PaO2、高PCO2使发生ROP的危险性增加。

Abstract:Objective: To investigate the effect of the value of blood gas analysis within three days in retinopathy of prematurity(ROP). Methods: 396 preterm infants need screening ROP were included,the blood gases were collected in the first 3 days after birth,the blood gas were analyzed. Results: The rates of preterm infants with ROP were much higher in the lower quartile PaO2,pH value and the upper quartile PaO2,PCO2(P<0.05).Using logistic regression analysis to analyse birth weight,gestational age,and blood gas for infants with severe ROP.We found that birth weight less than 1000g,gestatinal age less than 27 weeks,upper quartile PaO2,PCO2,lower quartile PaO2,and lower quartile pH with significant difference,and indicated that were the high risk of ROP. Conclusion: Preterm infants exposed to low PaO2,PH and high PaO2,PCO2 appear to be at increased risk of ROP.
