40Hz听觉相关电位评价聋哑儿童低频听阈及睡眠对其影响的探讨

摘    要:将64名聋哑儿童分为清醒组(A组)、睡眠组(B组)和自身对照组(C组),用0.5kHz及1kHz短音刺激,测得40Hz听觉相关电位(AERP)反应阈,并与听性脑干反应阈比较。发现:用115dB peSPL短声刺激未能引出ABR的73耳中,用0.5kHz短音刺激引出40Hz AERP者55耳,用1kHz短音刺激引出40Hz AERP者55耳。表明40Hz AERP对测试聋哑儿童低频听阈较为有效。比较A、B两组的40Hz AERP诱出率,无显著差异(P>0.05)。C组的清醒与睡眠状态40Hz AERP诱出率自身比较:刺激音为0.5kHz或1kHz短音时,睡眠组40Hz AERP诱出率明显高于清醒组(P<0.0001)。表明睡眠对测试儿童40Hz AERP反应阈无明显不良影响。当聋儿不合作时,用药物催眠后,由于信噪比改善,40Hz AERP的诱出率明显提高,这为40Hz AERP能在聋儿测听中广泛应用创造了条件。

Abstract:The response threshold of 40 Hz AERP was recorded in 64 deaf-mute children with 0.5 kHz and 1 kHz tone-pips stimulation,who were divided into three groups(group A:consciousness,group B:sleeping, group C:auto-control). The response threshold of ABR was not evoked with 115 dB peSPL in 73 ears,but in 55 ears 40 Hz AERP was evoked with 0. 5 kHz tone-pip and in 55 ears 40 Hz AERP was evoked with 1 kHz tone-pip.It indicated that 40 Hz AERP is more sensitive than ABR in testing the threshold of deaf-mute children especially in lower frequency hearing threshold within 0.5 kHz to 1 kHz.The evoked rate of 40 Hz AERP of group A and that of group B,were not very different from each other(P>0.05).The evoked rate of 40 Hz AERP in group C indicated that,sleeping had no significant poor effect on testing 40 Hz AERP,When inocrperative,they are hypnosed with medicine;thus the signal-to-noise ratio is improved and the evoked rate of 40 Hz AERP is remarkably higher(P<0.0001).This is of some value for wide application of 40 Hz AERP in testing audition of deaf children.
