| 摘 要： | 目的探讨实施连续性护理干预对出院高血压患者自我管理能力的影响。方法成立连续性护理干预小组,发放高血压自我管理手册,对患者进行健康教育,适时进行电话或家庭随访及健康讲座。结果提高了患者饮食、治疗、躯体活动自我管理能力,并在一定程度上控制了血压水平(P<0.01或P<0.05)。结论连续性护理能提高出院高血压患者的饮食、治疗、躯体活动自我管理能力,达到控制患者血压的目的。 |

| Abstract: | Objective To explore the effects of continuous nursing intervention on self - management ability among hypertension outpatients. Methods Measures included establishing the continuous nursing intervention group,providing self - management manual of hypertension, health education, proper telephone follow - up and delivering health lectures. Results After intervention,the patients＇ability of serf- management on diet,therapy, exercise were improved, and blood pressure was controlled to a certain degree （ P 〈 0.01 or P 〈 0.05 ）. Conclusion The continuous nursing can improve ability of self - management on di- et ,therapy,exercise and control blood pressure at some extent for hypertension outpatient. |
