0.33%罗哌卡因用于颈丛阻滞麻醉的临床观察

目的:比较0.33%罗哌卡因与1%利多卡因用于颈丛阻滞麻醉的效果和不良反应发生情况。方法:选择ASAⅠ～Ⅱ级的甲状腺次全切除手术患者80例,随机分为罗哌卡因组(R组)和利多卡因组(L组),分别使用0.33%罗哌卡因和1%利多卡因各30 ml,行双侧颈丛阻滞(一点法即C4横突深丛浅丛)。评价麻醉效果和麻醉后对心血管系统的影响,观察心率、血压变化及使用血管药物情况。结果:两组麻醉效果差异无统计学意义(P>0.05),麻醉后R组发生心动过速、血压增高,使用艾司洛尔及硝酸甘油的比例明显低于L组(P<0.05)。结论:罗哌卡因用于颈丛阻滞麻醉效果确切,对心率和血压的影响较利多卡因轻微,在颈丛阻滞麻醉中更具安全性和可行性。

Clinical observation of 0.33% ropivacaine in the block anesthesia of cervical plexus

Objective To compare the effect and side effect of 0.33% ropivacaine with 1% Lidocaine in the block anesthesia of cervical plexus.Method Choosing the level of ASAⅠ～Ⅱ and the thyroid gland inferior total excision surgery patients of 80 cases,were divided into ropivacaine group（group R） and Lidocaine group（group L）,using 0.33% ropivacaine and 1% Lidocaine of 30 ml,respectively,to block anaesthetize the bilateral of cervical plexus（the C4 deep plexus and light plexus）.The anaesthesia effect and the influence to cardiovascular system after anaesthesia were appraised,the heart rate and the blood pressure changes,use of blood vessel medicines were observed.Results Compared between the two groups,the effect of anaesthesia did not have the prominent difference（P0.05）,but after anaesthesia,group R showed tachycardia and the rise of blood pressure,the proportion of useing Siluo and the blasting oil were lower than group L obviously（P0.05）.Conclusion Compares with Lidocaine,ropivacaine in the block anesthesia of cervical plexus has accurate effect,its influence to the heart rate and the blood pressure is also slight,it is more security and feasibility when uses in the block anesthesia of cervical plexus.
