48例十二指肠溃疡急性穿孔临床分析

目的：探讨行单纯修补术及术后药物治疗十二指肠溃疡急性穿孔效果。方法：对我院2005年6月～2008年12月收治的十二指肠球部渍疡急性穿孔48例患者中46例实施单纯修补，术后给予抗溃疡药物及根除幽门螺旋杆菌（Hp）治疗，予以保守治疗2例。结果：近期效果良好，腹膜炎治愈，溃疡症状消失，胃镜检查示溃疡愈合，无死亡。术后45例随访0．5～3年，溃疡复发6例。结论：行单纯修补术治疗十二指肠溃疡急性穿孔疗效确切，术后行正规抗溃疡及根除Hp治疗能有效降低溃疡复发率和并发症的发生率。

Clinical analysis of 48 cases with acute perforated duodenal ulcer

Objective: To investigate therapeutic effect of simple closure operation and drug treatment on acute perforated duodenal ulcer.Methods: Retropectively analyzed clinical datas of 48 cases with acute perforated duodenal ulcer in our hospital during June 2005 to December 2008.46 cases had simple closure surgery,and were treated with anti-ulcer drug and anti-helicobacter pylori(Hp) therapy after operation;while 2 cases had conservative treatment.Results: It showed effective in short term.Peritonitis was cured,and all symptoms of ulcer disappeared.Gastroscopy displayed cured ulcer,and no case was dead.Follow-up of 45 cases,with 0.5-3 years duration,showed 6 cases had ulcer recurrence.Conclusions: Simple closure operation is effective on acute perforated duodenal ulcer.Normal anti-ulcer and Hp therapy can effectively decrease recurrence rate of ulcer and incidence rate of complication.
