分析剪切波超声弹性成像在乳腺良恶性肿瘤诊断中的应用价值

摘    要:目的:探讨分析剪切波超声弹性成像在乳腺良恶性肿瘤诊断中的应用价值。方法:选取本院治疗的70例乳腺占位性病变患者,共79个病灶,所有患者均进行彩超、剪切波超声弹性成像技术检查,所有病灶均经术后病理诊断为金标准。评价剪切波超声弹性成像诊断乳腺肿块良恶性的敏感性、特异性。结果:剪切波超声弹性成像诊断恶性44例,良性35例。良性病灶中,11个病灶(31.43%)最大弹性值14~42kPa,20个病灶(57.14%)最大弹性值42~59kPa,3个病灶(8.57%)最大弹性值60~68kPa,1个病灶(2.86%)最大弹性值68~70kPa。恶性病灶中,6个病灶(13.64%)最大弹性值71~147kPa,13个病灶(29.55%)最大弹性值148~241kPa,25个病灶(56.82%)最大弹性值242~300kPa。与病理结果对照,其诊断敏感度93.33%,特异度为94.12%,准确性为93.67%,误诊率5.88%,漏诊率6.67%,与病理诊断符合率较高(P>0.05)。结论:利用剪切波超声弹性成像辅助彩超诊断,可提高良恶性乳腺疾病诊断、鉴别准确率,是一种可靠的辅助诊断技术。

Abstract:Objective:To explore the value of shear wave ultrasound elastography in diagnosis of benign and malignant breast tumors.Methods:A total of 79 patients with mammary gland occupying lesions treated in our hospital were selected.All patients were examined by color Doppler ultrasound and shear wave ultrasound elastography.All the lesions were diagnosed as the gold standard by postoperative pathological diagnosis.To evaluate the sensitivity and specificity of shear wave ultrasound elastography in the diagnosis of benign and malignant breast masses.Results:44 cases of malignant and 35 cases of benign were diagnosed by shear wave ultrasound elastography.Among benign lesions,the maximum elasticity of 11 lesions(31.43%)was 14 to 42 kPa,the maximum elasticity of 20 lesions(57.14%)was 42 to 59 kPa,and the maximum elasticity of 3 lesions(8.57%)was 60 to 68 kPa.The maximum elasticity of each lesion(2.86%)was 68~70 kPa.Among the malignant lesions,the maximum elasticity of 6 lesions(13.64%)was 71~147 kPa,the maximum elasticity of 13 lesions(29.55%)was 148~241 kPa,and the maximum elasticity of 25 lesions(56.82%)was 242~300 kPa.Compared with pathological results,the diagnostic sensitivity was 93.33%,the specificity was 94.12%,the accuracy was 93.67%,the misdiagnosis rate was 5.88%,the missed diagnosis rate was 6.67%,and the coincidence rate with pathological diagnosis was high(P>0.05).Conclusion:The use of shear wave ultrasound elastography to assist color Doppler ultrasound diagnosis can improve the accuracy of diagnosis and differentiation of benign and malignant breast diseases,and is a reliable auxiliary diagnostic technique.
