老年慢性病患者下呼吸道院内感染病原菌分布及耐药性监测

目的：分析老年慢性病患者下呼吸道院内感染病原菌分布及耐药情况。方法:按常规方法进行细菌培养鉴定；药敏试验，耐药性监测按美国NCCLS推荐的操作标准进行。结果：202例老年病人均患有各种慢性消耗性疾病住院后所致的下呼吸道院内感染。在检出的37种250株菌中，革兰氏阴性杆菌（G-b）144株，酵母样真菌62株，革兰氏阳性球菌（G＋C）44株，其所占比例分别为55%，28%,17%。G-杆菌中大肠埃希氏菌，克雷伯氏菌属ESBL的阳性率分别为37.8%，39.6%；葡萄球菌属中MRS阳性率为67%；耐万古霉素肠球菌为6.0%。结论：老年慢性疾病患者是条件致病菌主要入侵对象，也是院内感染的高危人群，医院应重视与加强对这一特殊人群的护理与防卫措施。

Distribution of respiratory nosocomial infection pathogen on geratic peoplewith chronic diseases and its antibiotic resistance

Objective To detect respiratory nosocomial infection pathogen on geratic people with chronic disease and its antibiotic resistance.Methods Standard culture and antibiotic resistance method was used.Results Two hundred and fifty strains of species germ which caused nosocomial infection were cultured in 202 geratic people with chronic diseases,of which 144 strains were gram negative bacill (55%); 44 strains were gram positive coccus(17%); and 62 strains were Yeastlike Fungi (28%).The ESBL positive rate for Escherichiacol and Klebsiella was 37.8% and 39.6%.MRS positive rate for staphylococcus was 67%.Vancomycin resistance in Enterococcus was 6%.Conclusion Geratic people with chronic disease are more easily to infect opportunistic pathogen and cause nosocomical infection. The more care we give for geratic people,the lower the nosocomial infection rate.
