强力霉素治疗钩端螺旋体病研究

强力霉素(DOX)治疗早期钩端螺旋体(钩体)病的研究,国内尚未见报告。随机采用DOX治疗钩体病19例,并以青霉素G(PNC-G)治疗15例作对照。两组病例都治愈,其主要征候持续时间无显著差异。但PNC-G组发生赫氏反应(JHR)11例(73.3％),其中1例(9.1％)演变成肺弥漫性出血,而DOX组无JHR发生。DOX和PNC-G对新分离出的8株钩体的体外抗菌活性良好。检测3例DOX组病人的血药浓度,服药期中都显著高于最低抑菌浓度。

Clinical Trail of Doxycycline in the Treatment of Leptospirosis

Thirty-four cases of early leptospirosis was randomly divided into doxycycline (DOX) therapeutic group(19 cases)and penicillin-Gtherapeutic control group(15 cases). All patients were cured and it was no significantly different in efficacy between two groups. In penicillin-G group, Jarisch-Herxheimer reaction (JHR)occurred to 73. 3% (11 of 15)patients in which pulmonary diffuse hemorrhage(PDH)was induced in one case (1 of 11, 9. 1%). However,neither JHR nor PDH was found in DOX group. The blood level of DOX,determined in three cases by microbiological icdl method,was consistently more than the MIC of DOX against the pathogen during the therapy.
