胸腔镜辅助小切口治疗青少年自发性气胸

目的探讨电视胸腔镜辅助小切口（VAMT）治疗青少年自发性气胸的可行性及优越性。方法回顾性分析103例青少年自发性气胸患者的临床资料，其中56例采用电视胸腔镜辅助小切口手工切除、缝合或缝扎肺大疱的方法（辅助小切口组），47例采用常规电视胸腔镜Endo—GIA切除肺大疱的方法（胸腔镜组），对两组术中术后情况及围手术期并发症情况进行统计分析。结果两组手术失血量、手术时间、术后疼痛评分、下床活动时间、抗生素使用天数、留管时间、术后住院天数、肺部感染的差异均无统计学意义（均P〉0．05），但辅助小切口组住院费用低于胸腔镜组（P〈0．05）。结论VAMT肺大疱切除术操作简单、快捷，效果可靠，降低了住院费用。

Investigation on video-assisted minithoracotomy surgery in treatment of adolescent patients with primary spontaneous pneumothorax

Objective To investigate the feasibility and superiority of video-assisted minithoracotomy surgery (VAMT) in treatment of adolescent patients with primary spontaneous pneumothorax. Methods The clinical data of 103 adolescent patients with spontaneous pneumothorax were analyzed retrospectively. VAMTS was performed in 56 cases and video-assisted thoracoseopic surgery(VATS) was performed in 47 cases. complications Statistical analysis were undergoing in middle-operation, post-operation and ambi-operation in the two groups were analyzed. Results No statistically significant difference were found in blood loss ,operating time,pain grade, out-of-bed activity time, antibiotics use time,chest drainage time,hospital day pulmonary infection between the two groups( all P ＞0.05 ). The cost of hospitalization was lower in VAMTS group compare to VATS group ( P ＜ 0.05). Conclusion VAMT in treatment of adolescent patients with primary spontaneous pneumothorax had the advantage of simple performance, reliable effectiveness, practical,lower cost of hospitalization fee.
