超声造影在胆囊息肉样病变中的临床价值研究

目的探讨超声造影在胆囊息肉样病变临床治疗方式选择中的指导价值。方法回顾总结57例胆囊息肉样病变患者的常规超声、彩色多普勒及超声造影图像特征,分析病变血供对临床治疗方式的影响。结果38例超声造影显示病变血供不丰富,病变与周边胆囊壁呈同步增强,患者采取了随访观察;19例超声造影提示血供丰富,表现为快速增强、快速廓清,患者行手术治疗,术后病理显示9例为胆固醇息肉,6例为腺肌增生症,4为早期胆囊癌,其中2例伴有肝转移。结论超声造影对胆囊息肉样病变的诊断具有重要的意义,对其临床治疗方式的选择具有重要的参考价值。

The Clinic Value Study of Contrast-enhanced Ultrasonography in the Gallbladder Polyp Disease

Objective To explore the guidance value of contrast-enhanced ultrasonograpby in the clinical treatment option of gallbladder polyp disease. Methods We retrospectively reviewed the images characteristics of conventional ultrasound, color Doppler and contrast-enhanced ultrasound of 57 cases with gallbladder polyp disease and analyzed the influence of the lesions blood supply on clinical treatment option. Results Contrast-enhanced ultrasonography showed that the blood supply of lesions were not rich in 38 cases. The gallbladder polyp disease and its surrounding wall were simultaneously enhanced in these patients and all cases had follow-up. Contrast-enhanced ultrasonography displayed rich blood supply in 19 patients, and the enhanced performance was fast enhancement and rapid clearance. These patients had operations. Postoperative pathology showed 9 cases for cholesterol polyps, 6 cases adenomyomatosis, 4 for early gallbladder cancer, including two liver metastasis cases. Conclusion Contrast-enhanced ultrasonography has an important reference value in the diagnosis of gallbladder polyp lesions and an important reference value in the choice of clinical treatment.
