呼气末正压递增法肺复张治疗急性呼吸窘迫综合征的临床分析

摘    要:目的 探讨呼气末正压(PEEP)递增法肺复张(RM)治疗急性呼吸窘迫综合征(ARDS)的效果.方法 选择我院ICU病房收治ARDS患者42例,气管插管或气管切开接呼吸机,采用PEEP递增法实施RM,全程监测呼吸频率(RR)、呼吸系统顺应性(Crs)、平台压(Pplate)、心率(HR)、中心静脉压(CVP)、外周动脉平均压(ABPM);血气分析监测动脉血氧分压(PaO2)、血氧饱和度(SaO2)、血二氧化碳分压(PaCO2),计算氧合指数(PaO2/FiO2).结果 氧动力学指标:PaO2实施RM后10分钟(79.3±24.1)mmHg、30分钟(80.5±28.2)mmHg、1小时(82.7±26.3)mmHg、4小时(84.1±25.9)mmHg、24小时(87.6±23.2)mmHg与实施RM前(87.6±23.2)mmHg比较差异均有统计学意义(P<0.01);SaO2实施RM后10分钟(92±4)%、30分钟(93±3)%、1小时(94±3)%、4小时(96±3)%、24小时(97±3)%与实施RM前(88±6)%比较差异均有统计学意义(P<0.01);PaO2/FiO2实施RM后10分钟(134.6±40.1)mmHg、30分钟(142.8±37.4)mmHg、1小时(152.3±38.1)mmHg、4小时(163.5±34.2)mmHg、24小时(176.4±41.5)mmHg、与实施RM前(101.8±38.2)mmHg比较差异均有统计学意义(P<0.01).呼吸力学指标:RR实施RM后10分钟(31.8±4.9)次/min、30分钟(30.4±5.2)次/min、1小时(29.6±5.4)次/min、4小时(30.2±4.8)次/min、24小时(28.8±5.0)次/min与实施RM前(34.6±5.3)次/min比较差异均有统计学意义(P<0.05);Crs实施RM后10分钟(34.83±5.68)ml/cmH2O、30分钟(36.69±6.82)ml/cmH2O、1小时(38.43±7.02)ml/cmH2O、4小时(39.06±7.18)ml/cmH2O、24小时(40.12±7.38)ml/cmH2O与实施RM前(32.26±4.82)ml/cmH2O比较差异均有统计学意义(P<0.05);Pplate实施RM后10分钟(24.2±3.2)cmH2O、30分钟(23.8±2.8)cmH2O、1小时(23.2±3.0)cmH2O、4小时(22.6±3.3)cmH2O、24小时(21.3±2.5)cmH2O实施RM前(25.9±3.8)cmH2O比较差异均有统计学意义(P<0.01).血流动力学指标:HR、CVP、ABPM两组差异无统计学意义(P>0.05).结论 PEEP递增法RM改善肺部氧和情况的同时,不增加二氧化碳蓄积,顺应性可以得到一定程度的改善,能够减少呼吸功耗,降低吸入氧浓度,防止呼吸机相关性肺损伤.

Abstract:Objective To explore the effects of recruitment maneuvers(RM) with the method of gradual increase positive end-expiratory pressure(PEEP) in patients suffering from acute respiratory distress syndrome(ARDS).Methods Forty-two patients with ARDS in intensive care unit(ICU) of the hospital were enrolled in the study.All patients received protective ventilation by tracheal intubation or tracheotomy.RM was conducted by use of the method of gradual increase PEEP.Monitor was focused on the entire process and respiratory rate(RR),compliance of respiratory system(Crs),plateau pressure(Pplate),heart rate(HR),central venous pressure(CVP),mean of arterial blood pressure(ABPM);arterial partial pressure of oxygen(PaO2),blood oxygen saturation(SaO2),arterial carbondioxide partial pressure(PaCO2) were recorded and PaO2/Fi O2 was calculated.Results In dynamic indicators of oxygen:statistical significance existed in PaO2,after RM 10 minutes(79.3±24.1) mmHg,30 minutes(80.5±28.2) mmHg,1 hour(82.7±26.3) mmHg,4 hours(84.1±25.9) mmHg,and 24 hours(87.6±23.2) mmHg was compared with before RM(87.6±23.2) mmHg.Statistical significance existed in SaO2,after RM 10 minutes(92±4)%,30 minutes(93±3)%,1 hour(94±3)%,4 hours(96±3)%,and 24 hours(97±3)% was compared with before RM(88±6)%.Statistical significance existed in PaO2/FiO2,after RM 10 minutes(134.6±40.1) mmHg,30 minutes(142.8±37.4) mmHg,1 hour(152.3±38.1) mmHg,4 hours(163.5±34.2) mmHg,and 24 hours(176.4±41.5) mmHg was compared with before RM(101.8±38.2) mmHg.In respiratory mechanics:statistical significance existed in RR,after RM 10 minutes(31.8±4.9) times/min,30 minutes(30.4±5.2) times/min,1 hour(29.6±5.4) times/min,4 hours(30.2±4.8) times/min,and 24 hours(28.8±5.0) times/min was compared with before RM(34.6±5.3) times/min.Statistical significance existed in Crs,after RM 10 minutes(34.83±5.68) ml/cmH2O,30 minutes(36.69±6.82) ml/cmH2O,1 hour(38.43±7.02) ml/cmH2O,4 hours(39.06±7.18) ml/cmH2O,and 24 hours(40.12±7.38) ml/cmH2O was compared with before RM(32.26±4.82) ml/cmH2O.Statistical significance existed in Pplate,after RM 10 minutes(24.2±3.2) cmH2O,30 minutes(23.8±2.8) cmH2O,1 hour(23.2±3.2) cmH2O,4 hours(22.6±3.3) cmH2O,and 24 hours(21.3±2.5) cmH2O was compared with before RM(25.9±3.8) cmH2O.There was no statistical significance in hemodynamics:HR,CVP and ABPM(P〉0.05).Conclusion The method of gradual increase PEEP can not increase the accumulation of carbon dioxide while it improves the situation in lung oxygen.But it can improve compliance to some extent,reduce the respiratory power and the concentration of inhaled oxygen,prevent ventilator-associated lung injury.
