TK1在乳腺导管癌中的表达及其临床病理学意义分析

摘    要:目的检测胸苷激酶-1(Thymidine Kinase-1,TK1)在乳腺导管癌的表达情况,并分析与Ki67、ER、PR、C-erbB-2等临床病理学指标的关系,以及其与患者预后的关系。方法收集40例乳腺浸润性导管癌(invasive ductal carci-noma,IDC)组织,其中31例含有导管原位癌(ductal carcinoma in situ,DCIS)的成分,37例癌旁可见正常的乳腺组织(normal breast tissues,NBT)。采用免疫组织化学的方法检测40例乳腺癌组织中TK1、Ki67、ER、PR、C-erbB-2。结果 TK1在乳腺癌组织中浸润性导管癌的表达率为57.5%(23/40),导管原位癌为51.6%(16/31),癌旁正常的乳腺组织为0%(0/37),三者之间的表达率差异有统计学意义(P<0.001)。TK1表达与年龄、月经、肿瘤大小、淋巴结转移与否、TNM分期、ER、PR、C-erbB-2无关(P>0.05),与组织学分级有关(P<0.05)。TK1在浸润性导管癌Ⅰ级的表达率为20%(2/10),Ⅱ级为76%(19/25),Ⅲ级为40%(2/5),以组织学Ⅱ级为最高。TK1的表达与Ki67相关(r=0.738,P<0.001)。TK1阳性组的累积生存率低于TK1阴性组,并且TK1和Ki67均阳性的为最低。结论 TK1在乳腺癌组织中高表达,对于导管原位癌的诊断具有辅助价值。TK1是患者预后不良的一项指标。

Abstract:Objective To study the expression of TK1 in breast ductal carcinoma. To investigate the correlation of TK1 with Ki67, ER, PR, C erB-2, the clinical factors and pathological factors in breast cancer. To analyze the effect of TK1 and Ki67 on survival rate of breast carcinoma. Methods TK1, Ki67, ER, PR, and C erbB-2 were detected in 40 cases of breast carcinoma with immunohistoehemical staining. Results The expression rate of TK1 was 57.5% (23/40) in invasive duetal car cinoma (IDC), 51.6% (16/31) in ductal carcinoma in situ (DCIS), 0% (0/37) in normal breast tissues (NBT). There was significant difference between IDC, DCIS, and NBT (P〈0. 001). TK1 showed a correlation with tumor grade (P〈0.05), not with age, tumor size, lymph node metastasis, TNM stage, ER and PR, and C erbB-2. The expression rate of TK1 was highest in IDC grade Ⅱ (76%, 19/25), while the rate was 20% (2/10) in IDC grade I , and 40% (2/5) in IDC grade Ⅲ. The expression of TK1 correlated with Ki67 (r=0. 738, P〈0. 001). The cumulative survival rate of TK1 positive cases was lower than that of TK1 negtive cases and the rate of TK1&Ki67 positive cases was lowest. Conclusion TK1 expresses high in breast cancer, and it assists in the diagnosis of DCIS. TK1 is a mark of poor prognosis.
