42例淋巴瘤尸检多器官浸润病理变化研究

目的观察淋巴瘤对主要器官的浸润情况。方法对沈阳军区总医院1996年1月至2000年12月死亡的42例淋巴瘤患者进行尸体解剖,观察主要器官的大体变化及病理变化。结果42例尸检病例中有39例观察到肺部病理变化,占92.8%,主要表现为胸腔积液、胸膜炎症、肺播散、肺萎缩、肺出血、肺内感染、肺充血及水肿等。24例观察到心脏病理变化,占57.1%,主要表现为心包积液、心包及心壁播散。还有14例发生胃肠道播散,占33.2%,主要表现为肠穿孔,包括阑尾炎穿孔、回肠坏死穿孔及回肠癌性溃疡穿孔。另外,有少数病例同时出现多种病理变化。结论淋巴瘤可发生肺部、心脏及胃肠道等多器官浸润,本研究有助于对淋巴瘤患者出现呼吸功能障碍、心功能不全、肠梗阻及其他胃肠道症状的解释,对淋巴瘤的诊治有一定的临床指导意义。

Pathological features of organs infiltration of lymphoma in 42 autopsy cases

Objective To observe the pathological features of organs infiltration of lymphoma cells by autopsy in lymphoma patients.Methods The specimens and gross changes of different organs were acquired by autopsy in the 42 lymphoma patients.Results In the above-mentioned 42 autopsy cases,there were 39 cases(92.8%)with pathological changes including pleural effusion,pleurisy,lung disseminateion,pulmonary flaccidity,pneumonorrhagia,infection,congestion and dropsy,etc.The pathological changes in heart,which included hydrops pericardi,disseminateion in arcula cordis and paries,were observed in 24 cases(57.1%).Disseminateion in gastrointestinal tract was observed in 14 cases(33.2%),which displayed as intestinal perforation,including perforation related with appendicitis,cellular necrosis and cancerous ulcer of ileum.Furthermore,a few cases showed multi-changes in mutli-organs.Conclusion The lymphoma can infiltrate many organs,including lung,heart and gastrointestinal tract,which conduces to respiratory dysfunction,cardia insufficiency and gastrointestinal symptom.On the other hand,this study may be a clinical guidance.
