肾上腺少见病变的CT表现 (附12例报告)

目的探讨肾上腺少见病变的CT表现特点。方法2003年1月至2006年7月肾上腺少见占位性病变12例,其中男性7例,女性5例,年龄14～76岁,中位年龄42岁。回顾性分析12例经手术病理证实的肾上腺少见病变的CT资料。结果12例中,右肾上腺皮质癌2例,右肾上腺神经鞘瘤1例,左肾上腺节细胞神经瘤1例,多发内分泌肿瘤MEN-Ⅰ型1例,左肾上腺区巨淋巴结增生症1例,肾上腺髓样脂肪瘤4例5个病灶,肾上腺囊肿2例。囊肿及髓样脂肪瘤易于诊断;肾上腺皮质癌CT表现提示其为恶性;其他少见病变定性诊断困难。结论肾上腺少见病变均有一些CT表现特点;CT平扫加动态增强扫描对诊断有重要价值。

Analysis of CT manifestations in uncommon adrenal disorders:a report of 12 cases

Objective To study the characteristics of uncommon adrenal disorders with spiral CT scanning. Methods The spiral CT scanning datum of 12 patients surgical-pathologically proved uncommon adrenal disorders were analyzed retrospectively. Results In all 12 cases with uncommon adrenal disorder,including 2 cases of primary adrenocortical carcinoma at right adrenal gland, 1 case of right adrenal schwannoma, 1 case of left adrenal ganglioneuroma, 1 case of multiple endocrine neoplasia I , 1 case of Castleman disease in the area of left adrenal gland,4 cases of adrenal myelolipoma with 5 lesions and 2 cases of adrenal gland cyst.Adrenal myelolipoma and cyst were diagnosed easily;It should be malignancy if the CT imaging showed adrenocortical carcinoma;The nature of other uncommon adrenal disorder was difficult to recongnize. Conclusion It demonstrates there are certain characteristics in different uncommon adrenal disorders with spiral CT scanning. Combined and enhanced dynamic SCT scanning could pay an important role in diagnosis.
