全胃切除术后Lahey法及Lawrence法消化道重建术比较分析

目的:探讨Lahey法及Lawrence法消化道重建术对胃癌全胃切除术后患者营养状态及消化道症状的影响.方法:回顾性分析146例胃癌行全胃切除术的患者,其中以Lahey法重建者83例,以Lawrence法重建者63例,比较术后6个月时患者体重变化、营养状况以及消化道症状.结果:术后6个月Lawrence组体重下降程度显著低于Lahey组;血浆总蛋白、白蛋白水平变化,Lawrence组分别升高,而Lahey组分别降低.血红蛋白水平Lawrence组下降程度低于Lahey组,但无显著性差异.消化道症状,进食量、进食次数无显著性差异;呕吐者Lawrence组显著低于Lahey组.结论:Lawrence法消化道重建术是胃癌全胃切除术后较为合理的一种消化道重建方式.

The Comparision and Analysis between Lahey's and Lawrence's Reconstruction Procedures Following Total Gastrectomy

Objective: To explore the effects of Lahey's and Lawrence's reconstruction procedures on nutritional status and digestive symptoms following total gastrectomy for gastric cancer. Methods: To analysis retrospectively 146 patients performed total gastrectomy for gastric cancer. Among them, 83 cases were by Lahey's reconstruction procedures and 63 cases by Lawrence's. We focused on the comprision of the weight changing, the nutritional status and digestive symptoms in 6 months after operation. Results: In 6 months,the weight loss of Lawrence group was less than that of Lahey's significanly. For the level of total protein and albumin,they increased in Lawrence group, but decreased in Lahey's, and the difference was significant. The changing tredency for hemoglobin level in Lawrence group was less than that of Lahey group,but there was no significant difference.For the quantity and frequency of food intake, there was no significant difference. In Lawrence's, the number of patients with vomiting were less than that in Lahey's significantly. Conclusion: The Lawrence reconstruction procedure is a kind of reasonable ones following total gastrectomy for gastric cancer.
