石膏固定与外固定支架治疗老年性桡骨远端不稳定骨折疗效对比观察

目的对比石膏固定与外固定支架2种外固定方式治疗老年性桡骨远端不稳定骨折的疗效及临床应用价值。方法将80例老年桡骨远端不稳定骨折患者分为石膏组和支架组,每组40例。分别采用闭合复位加石膏外固定及闭合复位加外固定支架2种治疗方法。外固定均在骨折后2 h至3 d内进行,外固定时间6～8周,骨折愈合时间3～5月,随访6～24月。结果采用Sarmiento改良的Garland and Werley评分系统评定80例患者术后腕关节功能,石膏组的优良率为65%,支架组的优良率为90%;支架组优于石膏组（P〈0.05）。结论石膏固定与外固定支架2种外固定方式均为治疗老年性桡骨远端不稳定骨折的有效方法,外固定支架的疗效优于石膏固定。外固定支架治疗可避免石膏固定的卡压、血循环障碍、肌肉萎缩、受伤桡骨的短缩及腕关节僵硬等,单用外固定不适于Barton骨折的治疗。
