支原体肺炎实验室诊断及临床分析

目的探讨支原体（Mp）肺炎实验室诊断及临床特点。方法采用明胶颗粒凝集法（PA）检测呼吸道感染患儿血清Mp-Ig M,采用荧光定量聚合酶链反应（FQ-PCR）检测咽拭子肺炎支原体DNA,同时对53例肺炎患儿进行血清sVCAM-1、IL-6测定。结果Mp感染早期FQ-PCR检测Mp DNA的敏感性优于PA法检测Mp-Ig M,Mp肺炎组与非Mp肺炎组血清sVCAM-1浓度比较差异无统计学意义（t=2.66,P〉0.05）;Mp肺炎组血清IL-6浓度高于非Mp肺炎组（t=2.12,P〈0.05）。结论Mp DNA FQ-PCR检测可用于Mp感染早期诊断,其含量不受抗生素治疗的影响。IL-6可能参与Mp肺炎的炎症过程,其水平变化可以作为病情的判断指标。

Clinical analysis and laboratory diagnosis of Mycoplasma pneumonia

Objective To study the laboratory diagnosis and clinical characteristics of Mycoplasma pneumonia.Methods Serum Mp-IgM was measured by particle agglutination assay and the DNA of Mp in throat swabs was detected by FQ-PCR in children with respiratory tract infections.Serum sVCAM-1 and IL-6 were measured in 53 children with pneumonia.Results FQ-PCR sensitivity was superior to PA for early diagnosis of Mp infection.There were no significant differences in serum sVCAM-1 levels among the Mp group and the control group （t=2.66, P〉0.05）. The levels ofserum IL-6 in the Mp group were significantly higher those in the control group （t=2. 12, P〈0.05）. Conclusion Mp DNA FQPCR detection facilitated early diagnosis of Mp infection, and the content of Mp DNA was not affected by antibiotic therapy. IL-6 may be involved in the inflammation process and can serve as an indicator to help determine the severity of an infected individual＇s condition.
