江苏省包虫病流行病学调查

摘    要:目的了解江苏省包虫病流行的现状。方法对江苏省2005年以来的包虫病病例进行回顾性调查;对疑似本地感染的包虫病病人所在乡的宿主动物进行调查;ELISA检测病人所在乡镇的小学生、重点人群血清特异抗体和犬粪包虫抗原;血清阳性者作B超检查。结果共调查包虫病病例15例,其中4例疑似本地感染;犬和羊为主要中间宿主和终宿主;小学生、重点人群血清抗体阳性率、犬粪抗原阳性率分别为0.56%、0.22%和1.63%;B超检查未发现阳性病人。结论江苏省虽然出现了疑似本地感染包虫病病人、检出血清抗体阳性者和粪便抗原阳性犬,且存在完整的传播链,但未能找到病原体,因此不能判定江苏省为包虫病流行区,但也不能排除该可能性。

Abstract:Objective To understand the state of the hydatidosis epidemic in Jiangsu Province. Methods Hydatidosis cases after 2005 in Jiangsu Province were investigated. Definitive and intermediate hosts were also studied in villages of patients suspected of having hydatidosis. The serum of elementary school students and key populations and dogs feces in these villages were collected and assayed by enzyme-linked immunosorhent assay (ELISA). People with positive serum were examinated by B-mode ultrasound. Results Fifteen cases of hydatidosis were identified, and these included 4 cases of suspected infection. The main definitive and intermediate hosts were goats and dogs, respectively. Positive sera of ele- mentary school students and key populations were detected at rates of 0.56% and 0.22%, and dog feces was positive for antigens at a rate of 1.63%. B-mode ultrasound revealed that no patients were positive for hydatidosis. Conclusion Al- though there were suspected cases of primary hydatidosis, people with positive serum, dog feces positive for antigens, and there may even be a transmission chain, hydatidosis pathogens were not found. Thus, a conclusion could not be reached as to whether there exists a hydatidosis epidemic in Jiangsu Province.
