髋臼骨折的手术治疗

目的探讨髋臼骨折手术治疗的方法及临床效果。方法37例髋臼骨折，按Letournel分类：后壁骨折14例，后柱骨折5例，前柱骨折2例，横形骨折3例，T型骨折3例，后柱伴后壁骨折7例，双柱骨折3例。根据骨折类型采用前路、后路或前后联合入路，用重建钢板及螺钉内固定治疗。结果本组获得7-61个月（平均25个月）随访，按Matta评分标准，优14例，良21例，可2例。结论手术治疗是髋臼骨折治疗有效方法；手术入路选择，解剖复位，牢固固定及早期功能锻炼是治疗关键。

Surgical treatment of acetabular fracture

Objective To discuss the teatment of acetabular fracture and the curative effect. Methods 37 cases with aeetabular fracture Accirdubg to Letournel classification,there were 14 posterior wall fractures,5 posterior column fractures,2 anterior column fractures,3 transverse fractures,3 T-shaped fractures,7 posterior column fractures accompanying posterior wall fractures and 3 both column fractures. All cases with acetabular fractures were treated with pelvic reconstruction plates and compression screws on single or combined iliongui-nal. Or kocher-langenbeek approach. Results All cases were followed-up of 25 months. Their function of hip joint was evaluated by Matta criteria as excellent in 14 cases,good in 21 ,and fair in 2. Conclusion Surgical treatment is effective for the treatment of acetabular fracture. Beside firm fixation and early function exercise,the operation approach and reduction quality are the keys of sucees.
