穹顶形开窗治疗腰椎管狭窄症的临床研究

目的介绍穹顶形开窗术的临床应用，并进行临床疗效和影像学的评估。方法穹顶形开窗术治疗腰椎管狭窄症患者36例，平均年龄51．5岁，平均病程6年6个月。术后用改良的日本骨科学会下腰痛评分法(M—JOA)进行疗效评分和影像学检查。结果术后椎管横截面积较术前平均扩大2．3倍。术后M-JOA评分平均下降16．78分，优良率达94．4％。结论穹顶形开窗术治疗腰椎管狭窄近期疗效满意，既减压彻底，又减少对脊柱稳定性的破坏：

Domelike decompression of the lumbar vertebral canal in the treatment of lumbar spinal stenosis

Objective To describe the clinical applycation of Domelike decompr ession of the lumbar vertebral canal and evaluate the short-term outcomes. Metho ds Domelike decompression of the lumbar vertebral canal were performed in 36 pa tients,with an average age of 51.5 and an average history of 6 years and 6 month es. The surgical outcomes were evaluated with modify Japanese orthopedic associa tion low back pain score(M-JOA)and radiological findings. Results The average t ransverse area of vertebral canal was enlarged by 2.3 rates. The M-JOA average s core reduced 16.78 and the excellent and good ratio was 94.4%. Conclusion Short -term outcome of domelike decompression of the lumbar vertebral canal for lumbar spinal stenosis was satisfactory with thoroughly decompression and the lest d istroy of lumbar wertebral stability.
