血小板抗体的检测及其对血小板输注疗效的影响

目的：探讨血小板抗体的产生规律及其对血小板输注效果的影响。方法：检测A组（非过滤组）和B组（过滤组）患者血小板HLA抗体和HPA抗体，以CCI值评价两组患者血小板输注效果。结果：A组HLA抗体阳性率为53．33％（16／30）；HPA抗体阳性率为13．33％（4／30）；血小板输注无效率为43．33％（13／30）。B组HLA抗体阳性率为13．33％（4／30）；HPA抗体阳性率为10．00％（3／30）；血小板输注无效率为10．00％（3／30）。两组HLA抗体阳性率比较差异有统计学意义（P〈0．05），HPA抗体阳性率比较差异无统计学意义（P〉0．05），发生血小板输注无效比较差异有统计学意义（P〈0．05）。结论：输注滤除白细胞的机采血小板难以减少血小板HPA抗体同种免疫的发生率，但可以减少血小板HLA抗体同种免疫的发生率。提高血小板输注效果。

Platele Antibody Detection and Its Effect to Platelet Transfusion

Objective To discuss the occurrence rule ofplatelet antibody and its effect to platelet transfusion efficiency. Methods The patients were divided into two groups.Group B was transfused with leuco-reduced platelet and group A transfused without leuco-reduced platelet as control.To detect the positive rates of HLA antibody and HPA antibody of two groups and to evaluate the efficiency of platelet transfusion with platelet Corrected Count Increment（CCI）. Results The positive rates of HLA antibody and HPA antibody of Group A were 53.33%（16/30） and 13.33% （4/30） respectively, the rate of Platelet Transfusion Refractoriness （PTR） was 46.67% （14/30）.[n Group B, the positive rate of HLA antibody was 10.00% （3/30） and HPA antibody was 10.00% （3/30） respectively, the rate of PTR was 10.00% （3/30）. The difference in the postive rate of HLA antibody and the rate of PTR is statistically significant between the two groups （P 〈 0.05）. Conclusion Transfusion of leuco-reduced platelet can hardly reduce the occurrence of HPA antibody, but it can reduce the occurrence of HLA antibody and improve the effect of platelet transfusion.
