活血化瘀法治疗脓毒症的系统评价

目的 观察活血化瘀法对脓毒症患者白细胞（WBC）、血浆肿瘤坏死因子-α（TNF-α）、血浆白介素-6（IL-6）和28 d病死率的影响.方法 检索中国知网、维普、万方数据库查找2003年1月至2013年12月发表的关于活血化瘀法治疗脓毒症的相关研究文献,按照纳入与排除标准进行选择,并采用RevMan4.2.9进行Meta分析.结果 共纳入17项随机对照临床研究,治疗组共624例患者,对照组共612例患者.Meta分析的结果显示,与对照组相比,治疗组WBC计数降低WMD=-2.53,95％CI （-3.73,-1.33）],TNF-α浓度下降SMD=-1.73,95％CI（-2.31,-1.15）],IL-6浓度下降SMD=-1.68,95％CI （-2.28,-1.08）],28 d病死率下降OR=0.51,95％CI （0.35,0.72）].结论 活血化瘀法治疗脓毒症,能明显改善患者血浆WBC计数、TNF-α浓度、IL-6浓度和28 d病死率.

Systemic Assessment of Promoting Blood Circulation and Removing Blood Stasis in Treatment of Sepsis

Objective：To assess clinical efficacy of promoting blood circulation and removing blood stasis in treatment of sepsis by evaluating white blood cell （WBC）,serum level of tumor necrosis factor-alpha （TNF-α）,serum level of interleukin-6 （IL-6） and 28-day motality.Method：The related literature retrieval of promoting blood circulation and removing blood stasis in treating sepsis clinical trials （from January 2003 to December 2013） published was collected in multiple data-base （Chinese National Knowledge Infrastructure,VIP database,Wangfang Data）,and analysis by RevMan4.2.9 software.Result：17 researches were included finally,involving 624 patients in the treatment group and 612 patients in the control group.The Results of Meta analysis suggest that in comparison with the control group,the levels of WBC WMD=-2.53,95％CI （-3.73,-1.33）],TNF-α SMD=-1.73,95％ CI （-2.31,-1.15）],IL-6 SMD=-1.68,95％ CI （-2.28,-1.08）] and 28-day motality OR=0.51,95％ CI （0.35,0.72）] decreased remarkably in the treatment group.Conclusion：The Method of promoting blood circulation and removing blood stasis can significantly improve the levels of WBC,TNF-α and IL-6 and 28-day motality in patients with sepsis.
