新四联方案补救治疗幽门螺杆菌感染临床观察

目的：探讨含左氧氟沙星的新四联方案补救治疗幽门螺杆菌（坳）感染的疗效及安全性。方法：84例Hp感染初次三联疗法根除失败的消化性溃疡或慢性胃炎患者随机分成两组，治疗组44例，用左氧氟沙星0．2g，阿莫西林1．0g，枸橼酸铋钾0．22g，埃索美拉唑20mg po，bid，连服10d。对照组40例，用四环素1．0g，呋喃唑酮0．2g，胶体果胶铋0．24g，埃索美拉唑20mg po，bid，连服10d。4周后复查14^C—UBT。结果：治疗组和对照组补救治疗Hp感染的根除率分别为88．6％和62．5％（P〈0．01），不良反应发生率为6．8％和33％，差异均有统计学意义（P〈0．01）。结论：新四联方案为安全有效的抗Hp的补救治疗方案。

Clinincal Study of New Quadruple Regimen as Remedial Therapy on Helicobacter Pylori Infection

Objective： To explore the effect and safety of contained levofloxacin quadruple regimen on treatment of helicobacter pylori（Hp） infection. Method： Eighty-four peptic ulcer or chronic gastritis patients with a previous failed course of triple therapy were divided into two groups： test group （44patients） were given the new quadruple therapy：levofloxacin 0.2g bid, amoxicillin 1.0g bid plus potassimcitrate bismuth 0.22 g bid and esomeprazole 20mg bid; compare group （40patients） treated with tetracycline. 1.0g bid, furazolidon 0.2 bid,colloidal bismuth pectin （CBS） 0.24g bid, esomeprazole 20mg bid . The course of treatment was 10 days. After 4 weeks,Hp status was assessed by 14^C-UBT. Result： Eradication rates of Hp in the test group were 88.6% and in the compare group were 62.5% ;The side effect rate were 6.8% and 33% respectively. There were statistical difference between two groups（ P 〈 0.01 ）. Conclusion： Levofloxaein based quadruple regimen is an effective and safe remedial therapy after a failed first line triple therapy.
