糖尿病患者血糖与胃泌素、胃动素、生长抑素相互关系的初步探讨

摘    要:目的 探讨糖尿病患者血糖与胃泌素、胃动素、生长抑素相互关系。方法 测定糖尿病组(35例)、糖尿病胃轻瘫组(31例)和对照组(28例)空腹血糖(FBG)、胃泌素(GAS)、胃动素(MOT)、生长抑素(SS)浓度。结果 糖尿病组、糖尿病胃轻瘫组与对照组比较,GAS和MOT均有显著升高(P〈0.01),SS呈现不同程度的降低,但仅糖尿病胃轻瘫组有显著性差异(P〈0.01)。糖尿病胃轻瘫组与糖尿病组比较,GAS、MOT和SS均有不同程度的降低。但仅有SS降低显著(P〈0.05)。糖尿病组、糖尿病胃轻瘫组FBG均与GAS、MOT呈直线正相关(P〈0.05),糖尿病组相关系数1为0.467、0.486,胃轻瘫组为0.417、0.456;FBG与SS呈直线负相关(P〈0.05),糖尿病组相关系数γ为-0.477,胃轻瘫组γ为-0.480。糖尿病组和糖尿病胃轻瘫组GAS、MOT与SS呈直线负相关(P均〈0.05),糖尿病组相关系数γ分别为-0.425和-0.409。胃轻瘫组为-0.458和-0.431。结论 糖尿病、糖尿病胃轻瘫患者GAS、MOT水平升高,SS水平降低。可能的原因是高血糖使SS分泌减少。致使其对GAS、MOT的抑制作用减弱,GAS、MOT水平升高。

Abstract:Objective To study the correlation among plasma fasting blood glucose(FBG), gastrin(GAS), motilin(MOT) and somatostatin(SS) levels in diabetes mellitus(DM) and diabetic gastroparesis(DGP). Methods 35 patients with DM, 31 patients with DGP and 28 healthy volunteers were enrolled in the study, the GAS, MOT and SS levels were measured by using radioimmunoassay. Results the levels of GAS and MOT in DM and DGP were significantly higher than in the control group (P<0.01 for both), but the SS levels were lower, and just in DGP had statistical differences(P<0.05). There was positive correlation between the levels of GAS, MOT and FBG in DM and DGP(P<0.05 for all), while negative correlation between the levels of SS and FBG(P<0.05). There was negative correlation between GAS, MOT and SS in DM and DGP(P<0.05 for all). Conclusions The levels of GAS, MOT were higher and SS were lower in DM and DGP than in the control group. The reason might be that hyperglycemia result in gastroenteric hormone secretion unbalance.
