乳腺彩超和临床检查的一致性及预后的研究

摘    要:目的统计分析乳腺彩超发现低回声结节与临床查体的一致性,探讨不同低回声结节处理及预后的特点。方法选取海淀妇幼保健院乳腺病防治中心2009年5~12月间常规采用乳腺彩超进行乳腺癌筛查患者,共1456人,采用美国放射学会(ACR)制定的乳腺影像报告和数据系统(B I-RADS)的标准将乳腺彩超结果进行分级,统计分析乳腺彩超下直径大于≥1cm组及<1cm组两组临床查体以及处理的差别。结果 1456人中乳腺彩超探及低回声结节共244人,占16.76%,乳腺彩超下低回声结节直径大于≥1cm组的临床查体检出率为28.60%,<1cm组临床查体检出率为7.5%,两者比较2χ=19.445,P=0.000,差异有显著意义;乳腺彩超下低回声结节直径大于≥1cm组的活检率33.30%,<1cm组的活检率为18.80%,两者比较2χ=6.465,P=0.011,差异有统计学意义。结论乳腺彩超发现低回声结节的敏感性较高,<1cm的低回声结节临床查体检出率较低,其中有18.80%的结节B I-RADS-US分级为4级及以上,需要活检,而这部分需活检者有患恶性肿瘤的可能,需要临床医生在临床工作中警惕这部分病例,注重与乳腺彩超检查相结合,减少乳腺疾病的漏诊。
