放化疗联合治疗老年局部晚期鼻咽癌患者疗效观察

目的观察老年局部晚期鼻咽癌患者的单纯放疗和放疗联合化疗的疗效及毒副作用。方法72例老年局部晚期鼻咽癌患者,其中单纯放疗组34例,放疗联合化疗(放化疗)组38例,比较两组的疗效及毒副作用。结果单纯放疗组和放化疗组的鼻咽全消率、颈残留率分别为85.71％、22.86％和91.89％、10.81％,P>0.05,无统计学意义。3年生存率分别为47.50％、和55.17％,差异无统计学意义,P>0.05。两组毒副作用差异有统计学意义,放化疗组胃肠道反应、骨髓抑制比单纯放疗组明显,P<0.05,单纯放疗组的复发、转移率比放化疗组明显升高,0.050>P>0.025。结论放疗联合化疗对老年局部晚期鼻咽癌的疗效提高不能完全肯定。

Combination of Treatment with Radiotherapy and Chemotherapy Old Patients with Locally Advanced Nasopharyngeal Carcinoma

Objective To observe the efficacy and side effects of pure radiotherapy and radiotherapy combined with chemotherapy to older patients with locally advanced nasopharyngeal carcinoma( LA-NPC).Methods Seventy-two cases of LA-NPC were analysed retrospectively and 34 cases among them were treated with pure radiotherapy,38 cases with radiotherapy combined with chemotherapy,the therapeutic and side effects were compared between them,respectively.Results The clearance rates of primary lesion in nasopharynx were 85.71% in pure group and 91.89% in union group,respectively P >0.05.The remain rates were 22.86% and 10.81%,respectively P>0.05.The three years survival rates were 47.50% and 55.17%,respectively P>0.05.The side effects including gastrointestinal response and bone marrow depression in union group was markedly higher than those in pure group P <0.05.The recurrence rates and metastasis rates of pure group was higher than union group,0.050>P>0.025.Condclusions Radiotherapy combined with chemotherapy may not increase the effect of LA-NPC of older patients compared with pure radiotherapy.
