血清肿瘤标记物及HSP60联合检测对大肠癌诊断的意义

目的 探讨血清肿瘤标记物CEA、CA19-9及HSP60的水平与大肠癌生物学特性的关系及联合诊断的价值.方法 采用电化学发光法及酶联免疫法分别对45例大肠癌患者和44例正常对照者进行血清中CEA、CA19-9及HSP60含量的检测,分析其与大肠癌转移和临床分期的关系,并用ROC曲线分析癌症患者血清中CEA、CA19-9及HSP60的联合诊断价值.结果 大肠癌患者血清中CEA、CA19-9及HSP60水平明显高于正常对照组,尤其在Ⅲ-Ⅳ期以及有淋巴转移的患者血清中水平更高,ROC曲线结果显示三项联合检测敏感性、特异性及AUC值可达到80.0%、90.9%和0.90,与单项或者二项联合相比,可明显提高诊断的准确性.结论 血清中CEA、CA19-9及HSP60水平是大肠癌发生、发展的重要检测指标,三项联合检测对大肠癌患者诊断和预后等有着重要意义.

Combined diagnosis value of serum tumor markers and HSP60 in patients with colorectal cancer

Objective To evaluate the combined diagnosis value of serum tumor marker CEA,CA19-9 and HSP60 and explore the relationships between them and biological characteristic of colorectal cancer. Methods 45 colorectal cancer patients and 44 normal people serum CEA,CA19-9 and HSP60 levels were detected by electrochemiluminescence and ELISA,the relationship between them and colorectal cancer metastasis and clinical staging were analyzed, and the combination diagnosis value of serum CEA, CA19-9 and HSP60 were assessed by ROC analysis. Results Colorectal cancer patient serum CEA,CA19-9 and HSP60 levels were obviously higher than those of control group, especially in patients with metastasis or at the III-IV stage. The ROC curve indicated the sensitivity, specificity and AUC value of 3- marker combination reached 80.0% ,90.9 % and 0.90,respectively. Compared with CEA and CA19-9 alone or CEA＋ CA19-9,CEA＋ CA19-9＋- HSP60 had higher diagnosis accuracy in colorectal cancer. Conclusion Serum CEA, CA19-9 and HSP60 levels were important factors for colorectal cancer genesis and development,and the 3-marker combination has important significance for colorectal cancer diagnosis and prognosis estimation.
