青少年原发性肝癌与乙型及丙型肝炎病毒感染的关系分析

目的研究我院收治的青少年原发性肝癌(PHC)患者乙型肝炎病毒(HBV)、丙型肝炎病毒(HCV)、HBV/HCV混合感染率,为采取有效的防治措施提供科学依据。方法收集2001年1月—2011年10月解放军302医院收治的青少年PHC患者为病例组,同期住院的其他肝病患者为对照组,对HBV、HCV、HBV/HCV血清标志物阳性率情况进行分析。结果病例组HBsAg、抗HBe、HBeAg、HBV-DNA阳性率分别为77.5%、55.0%、17.5%和40.0%,均高于对照组(25.0%、20.0%、5.0%和7.5%);抗HCV和HCV-RNA阳性率分别为12.5%和15.0%,HBV/HCV混合感染率为12.5%,均高于对照组(5.0%、2.5%和2.5%)。结论 HBV感染、HBV/HCV混合感染对青少年PHC患者的发病具有一定作用,但HBV感染仍是主要的因素。

Association between HBV and HCV Infections and Primary Hepatic Carcinoma in Adolescents

Objective To explore the association between hepatitis virus B(HBV) and hepatitis virus C(HCV) infections and primary hepatic carcinoma in adolescents.Methods A total of 40 adolescents with primary hepatic carcinoma who were admitted to the PLA 302 hospital from January 2001 to October 2011 were enrolled as the case group,and another 40 age-matched patients who were treated during the same period as the control group.The positive rates of serum markers including HBsAg,anti-HBe,HBeAg,HBV-DNA,anti-HCV,and HCV-RNA were detected.Results The positive rates of HBsAg,anti-HBe,HBeAg,and HBV-DNA in the case group were 77.5%,55.0%,17.5%,and 40.0%,respectively,which were significantly higher than those in the control group(25.0%,20.0%,5.0%,and 7.5%).The positive rates of anti-HCV,HCV-RNA,and HBV/HCV mixed infection were 12.5%,15.0%,and 12.5% in the case group,which were also significantly higher than those in the control group(5.0%,2.5%,and 2.5%).Conclusion Both HBV and HCV infection play a role in the development of primary hepatic carcinoma in adolescents,while HBV infection remains the most important factor.
