小儿喉尖锐湿疣29例临床分析

目的:提高对小儿喉尖锐湿疣(ILCA)的认识。方法:回顾分析我院1994-2004年29例ILCA患儿及其父母的临床资料。结果:患儿初发年龄在2个月-2岁,以渐进性声嘶加重、呼吸困难为首发症状,直达喉镜下见声门区、会厌喉面多发灰白色或淡红色乳头状或息肉状赘生物,组织病理检查为出现特征性的灶性分布凹空细胞,其中27例患儿其父母亲至少有一方曾患生殖器尖锐湿疣,所有病例均采用以手术切除为主的综合治疗,术后易复发,随诊2年以上23例,治愈15例(65.2%),好转6例(26.1%),死亡2例(8.7%)。结论:本病病变范围大,复发率高,预防的关键是及早发现及治愈孕妇尖锐湿疣,以减少新生儿接触的机会。

The Clinical Analysis of Larynx Condyloma Acuminatum in 29 Children Cases

Objective:To increase the knowledge of infantile laryngeal condyloma acuminatum(ILCA). Methods:The clinical data of 29 cases with ILCA and their parents from 1994 to 2004 in our hospital have been investigated. Results:The first onset age were 2 months to 2 years old,the first symptom were trachyphonia and dyspnea.The larynx side of epiglottis in vocal area glottis appeared gray and rufous puffiness with the shape of corpora mammillaria or polypoid hyperplasia,pathologic findings were hollow and concavity cell with focal distribution under the direct laryngoscope.There were 27 cases whose father or mother suffered genital condyloma acuminatum.The treatment for ILCA was exairesis.The relapse rate was higher,15 cases(65.2%)recovered,6 cases(26.1%)improved and 2 case(8.7%)died during 2 years follow-up.Conclusion:The ILCA was extensive and the relapse rate was higher,the key points of preventing ILCA were early diagnosis and treatment of pregnant woman with condyloma acuminatum in order to reduce the touch of newborn to the condyloma acuminatum.
