中医分型论治腹泻型肠易激综合征经验总结及分析

目的:归纳整理中医分型论治腹泻型肠易激综合征(IBS-D)的经验,并总结分析其临床常见证型。方法:利用中国期刊网(CNKI)数据库,通过主题词与自由词结合的方式,检索2007年8月至2017年8月10年间国内相关文献,对各文献提出的论治经验逐一统计、分类、分析。结果:近10年来,中医主要从6大方面论治本病,分别为从肝脾相关论治、从肝脾肾同调论治、从肝风论治、从情志论治、从脾胃升降论治以及从浊毒论治。结论:从上述论治经验可归纳出肝脾不调证、脾肾阳虚证、肝风内动证、心神不养证、脾胃不和证与浊毒内蕴证六大临床常见证型,临床辨证后分别施以肝脾同调法、肝脾肾同调法、柔肝息风法、养心安神法、辛开苦降法以及化浊解毒法治疗可取得显著疗效。

Summarization and Analysis of Experience in Treating Diarrhea-Predominant Irritable Bowel Syndrome Based on Syndrome Differentiation of Traditional Chinese Medicine

Objective:To summarize and analyze the experience in treating diarrhea-predominant irritable bowel syndrome(D-IBS) based on syndrome differentiation of traditional Chinese medicine(TCM),and to summarize the common syndrome types of D-IBS in clinic. Methods:Related literature from August 2007 to August 2017 were searched in the China National Knowledge Infrastructure(CNKI) by combing subject headings and free words,and the obtained literature was classified and analyzed. Results:In the last 10 years,TCM treats D-IBS mainly from 6 aspects:relationship between liver and spleen,regulating liver,spleen and kidneys,liver wind,emotions,ascending and descending of spleen and stomach qi,and turbid toxicity. Conclusion:From the above 6 aspects,it was concluded that there were 6 common syndromes in clinic:disharmony between liver and spleen,yang deficiency of spleen and kidney,internal stirring of liver wind,malnutrition of heart spirit,disharmony between spleen and stomach and internal accumulation of turbid toxicity.After syndrome differentiation,methods like regulating liver and spleen,regulating liver,spleen and kidneys,softening the liver and extinguishing wind,nourishing the heart and calming the mind,acridity opening and bitterness discharging,removing turbidity and resolving toxicity could be applied to obtain satisfactory effect.
