大承气汤联合丙胺酰-谷氨酰胺对早期重症急性胰腺炎疗效分析

目的:观察大承气汤联合丙胺酰-谷氨酰胺对早期重症急性胰腺炎的疗效。方法:选取早期重症急性胰腺炎患者106例,按随机数字表法分为2组,各53例。对照组采用常规治疗,观察组在对照组基础上以大承气汤联合丙胺酰-谷氨酰胺治疗。对比两组临床指标及治疗前后血清相关指标、健康评分变化。结果:观察组首次排便时间、肠道功能恢复时间、腹部不适缓解时间及住院时间均短于对照组,器官衰竭及感染发生率均低于对照组,差异有统计学意义(P0.05);观察组治疗3 d、7 d后肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)及二胺氧化酶(DAO)水平均低于对照组,差异有统计学意义(P0.05);观察组治疗7 d后急性生理与慢性健康(APACHEⅡ)评分及CT严重程度指数(CTSI)评分均低于对照组,差异有统计学意义(P0.05)。结论:大承气汤联合丙胺酰-谷氨酰胺用于治疗早期重症急性胰腺炎可有效改善患者临床指标及血清相关指标,并促进疾病康复。

Therapeutic Effect of Dachengqi Decoction Combined with Acetamido-glutamine on Severe Acute Pancreatitis

Objective:To investigate the effect of Dachengqi Decoction combined with acetamido-glutamine on severe acute pancreatitis. Methods:A total of 106 patients with severe acute pancreatitis were randomly divided into two groups,with 53 cases in each group. The control group was treated with conventional therapy,and the observation group was treated with Dachengqi Decoction in combination with alanyl-glutamine in the early stage. The clinical indicators of the two groups and the changes of serum related indicators and health scores before and after treatment were compared. Results:The first defecation time,intestinal function recovery time,abdominal discomfort relief time and hospitalization time were shorter in the observation group than those in the control group. The incidence of organ failure and infection of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05).The levels of TNF-α,IL-6 and DAO in the treatment group were lower than those in the control group at3 d and 7 d,the difference was statistically significant(P<0.05). The APACHE II score and CTSI score in the observation group were lower than those in the control group after 7 days of treatment. The difference was statistically significant(P<0.05). Conclusion:Dachengqi Decoction combined with alanyl-glutamine can effectively improve the clinical indicators and serum related indicators,and promote the rehabilitation of the disease in the treatment of severe acute pancreatitis.
