老年肺癌患者的临床分析

目的回顾分析老年肺癌患者的临床表现、分期、治疗方法和生存期。方法统计我院可随访老年肺癌患者56例,分析首发症状、临床分期、类型、治疗情况以及生存期。结果首发症状多为咳嗽、咳痰,胸闷气短,其中还有一部分患者是查体发现。其中鳞癌占30.4%,腺癌占46.4%,小细胞肺癌占21.4%。65~79岁组老年肺癌患者中位生存期12个月,≥80岁组老年肺癌患者中位生存期8个月。影响预后的重要因素是病变的临床分期和组织类型。结论老年肺癌患者首发症状不典型,病变的临床分期和组织类型对预后有影响,而年龄与生存期无关。晚期老年肺癌患者应以支持治疗为主。

Clinical study on senile patients with lung cancer

Objective To analyze retrospectively the clinical manifestation,stages, therapy methods and lifetime of senile patients with lung cancer. Methods 56 cases of elderly patients with lung cancer were followed up and their initial symptoms,clinical stages,treatments and lifetimes were investigated. Results Patients with initial symptoms including cough, expectoration accompanied with oppressed feeling in chest dominated in all patients and some were found in physical checking. Histological examination revealed that squamous cells carcinoma was accounted for 30.4% in all cases with 30.4% of adenocarcinoma and 21.4% of small cells lung cancer. The median of lifetime was 12 of months in the group of 65 - 79 years old patients and that of equal or above 80 - years - old was 8 of months. The major impacts on prognosis were clinical stages and histological styles. Conclusions Senile patients with lung cancer demonstrated atypical symptoms and their prognosis and lifetimes are related to clinical stages and histological styles with no relation to ages. Senile patients with later lung cancer should be treated by supportive therapy.
